Collection of Data by MySkillCheck Mobile Application

This application will be collecting data solely based off a unique identifier, otherwise known as MySkillCheck ID. The MySkillCheck contains the data of eligible or non-eligible only. The application in return will stamp where and when the MySkillCheck ID was scanned or entered int the system. The history will save this data of who was scanned, what the status is (eligible or non-eligible), the location, and time. You can report or filter the number of scans completed by day, week, month. The data does not store past the 1-month mark.

The uses of the data can later be used to produce reports representing the data captured above. The data will remain property of Mobile Medical Corporation only and will not be shared with outside parties. The data may be used for Mobile Medical Corporation clients of whom has appropriate permissions to access data related to that client.

Application data will only be stored for a period of 1 month. While online users may view a longer date range up to 1 year. Users may opt out of the program or data access or once removed as a Mobile Medical Corporation customer.

Declining the Collection of Personal Information

To the extent that Mobile Medical Corporation seeks personal information from you, you will be prompted to provide that information. However, you will always have the choice to decline to provide the information and not receive the services or materials from Mobile Medical Corporation.
